Mission Statement
Continuum of Care's mission is to enable people who are challenged with mental illnesses, intellectual disabilities and/or struggling with addiction to rebuild a meaningful life and thrive in the community. We do this by providing comprehensive residential housing & housing resource services as well as wrap-around support services, such as case management, crisis stabilization, training and employment opportunities, and psychiatric services to this population. Continuum of Care serves individuals in New Haven, Fairfield and Hartford Counties in Connecticut.
